There are many areas to consider in planning and building a good life for a young adult after high school. The best transition planning starts early, builds a team and support network around the young person and their family, and is person-centered.

Please join us for an evening of learning as we review the stages and steps involved in developing a plan for transitioning youth with developmental disabilities as they prepare for life after school.

This free session is suitable for parents of teens aged 15 and up and for professionals/educators who support transitioning youth who have a developmental disability.
